Notice Informing Individuals about Nondiscrimination and Accessibility Requirements

Hospice of the Panhandle, Inc., complies with applicable federal civil rights laws and does not discriminate on the grounds of race, color, national origin, legal status, age, disability, including discrimination based on pregnancy, sex, gender identity and sex stereotyping.

Hospice of the Panhandle, Inc., provides the following communication aids and services free of charge:

  • Qualified sign language interpreters;
  • Large print materials; and
  • Free interpretation services to those whose primary language is not English.

If you need these services, please contact your nurse or social worker or Anita Whitesides, Quality Improvement Coordinator.

If you believe that Hospice of the Panhandle, Inc., has failed to provide these services or discriminated against you on the grounds of race, color, national origin, legal status, age, disability, including discrimination based on pregnancy, sex, gender identity and sex stereotyping, you may file a grievance by in-person, by mail, telephone, fax or e-mail to:
